Community First Choice is a program designed to help Connecticut Medicaid members receive services at home. CFC emphasizes and supports a person’s ability to have an active role in their own care. Services can include:
- Meal preparation
- Household chores
- Assistance with daily activities (such as bathing, dressing, etc)
- Educational services to help you increase independence and manage in-home staff

Who is eligible for CFC?
A person needs to meet a certain level of need as determined by Medicaid; that means you would be eligible for hospital or nursing facility care. If you would qualify to live in a nursing facility, you will qualify for CFC. You also need to be able to self-direct, which means you take an active role in managing your care. CFC is set up to help you be in control of the assistance you receive.
What kinds of services does CFC provide?
CFC provides a wide range of services. This includes help preparing meals and doing household chores, and assistance with other activities as needed, such as bathing and dressing. Staff can also take you to appointments, run errands, and more.
Will I have a say in who works with me in my home?
Yes! This program lets you decide who works with you. If you enroll in CFC, you will be able to choose who to hire from a pool of qualified staff. You even have the option to hire certain family members and friends. You will set the hiring requirements for each of your staff.
How can I apply?
You can apply online or by phone. To complete an application by phone, dial 2-1-1 and choose Option 3. This line is available between 8:30 am and 5:00 pm Monday through Friday.

A CHCPE care manager will help you through the steps to figure out what’s best for you.
