The Connecticut Home Care Program for Disabled Adults (CHCPD), administered by the Department of Social Services, is a state-funded program that offers home and community-based services and supports to people with a primary diagnosis of a degenerative, neurological condition.
This pilot program is intended to serve people age 18 to 64 who are not financially eligible for Medicaid and need case management and other supportive services.
There is a limited availability for this program–a maximum of 100 people may receive CHCPD benefits.
