Home Care for People with Degenerative, Neurological Conditions

The Connecticut Home Care Program for Disabled Adults (CHCPD), administered by the Department of Social Services, is a state-funded program that offers home and community-based services and supports to people with a primary diagnosis of a degenerative, neurological condition.

This pilot program is intended to serve people age 18 to 64 who are not financially eligible for Medicaid and need case management and other supportive services.

There is a limited availability for this program–a maximum of 100 people may receive CHCPD benefits.

Eligibility Criteria

To be eligible for CHCPD, individuals must:

  • Be between the ages of 18 and 64
  • Have a diagnosis of a degenerative neurological disease
  • Require nursing facility level of care
  • Not be eligible for the Personal Care Assistance (PCA) waiver
  • Not be eligible for Medicaid and
  • Meet the financial requirements. There is no income limit, however asset limits in 2018 are $37,080 for an individual and $49,440 for a couple.

Some neurological conditions that may allow eligibility for CHCPD benefits, include but are not limited to:

  • Multiple Sclerosis
  • Alzheimer’s Disease
  • Parkinson’s Disease
  • Huntington’s Disease
  • Amyotrophic Lateral Sclerosis
  • Pick’s Disease
  • Creutzfeldt-Jakob Disease

People with a primary disability of mental illness or intellectual disability are not eligible to participate in this program.

Only 100 people may receive CHCPD during a given time. Due to the limited number of available slots, waiting lists for the CHCPD program are common. The waiting list is based on a first come, first served basis. Department of Social Services staff will notify individuals on the waiting list when a slot becomes available.
