Home health care helps people recover from illness or injury and remain in their home by bringing services and supports into the home. Prescribed by an individual’s physician, home health care is part of a written plan of care.

While it can include non-medical services, the primary focus is medical and includes rehabilitation services to improve the person’s condition and help them regain their independence. This service often follows hospitalization, rehab or skilled care in a nursing facility.

Home health care can be provided in tandem with other types of home care. Non-medical home care includes help with Activities of Daily Living (ADLs) such as transportation, meal preparation or getting dressed.

Eligibility for home health care services is determined by the individual’s physician.

Services and Fees

Home health care can include a range of medical services such as:

  • Rehabilitation services
  • Skilled nursing
  • Home health aides
  • Physical, occupational and/or speech therapy
  • Hospice

Costs of these services may be covered by Medicaid and Medicare.

Private Pay:
Private pay refers to using personal funds to cover all or a portion of an expense. There are different ways to acquire private pay resources.

Medicare:
This is the federal health insurance program for older adults, people with disabilities and people with End-Stage Renal (Kidney) Disease and ALS. Benefits can be received through different programs and policies either from the government or a private insurance company.

HUSKY Health and Medicaid:
This program provides a comprehensive health care benefit package including preventive care, primary care, specialist visits, hospital care, behavioral health services, dental services, hospice and prescription medications to eligible individuals.

Community First Choice (CFC):
Community First Choice (CFC) offers a variety of Services & Supports to help individuals meet their goals and to provide hands-on care and supervision in their home and community while self-directing staff.

Money Follows the Person Program (MFP):
This federal demonstration program helps individuals living in long-term care facilities and on Medicaid transition back into the community. A wide range of Services & Supports are available.

Katie Beckett Waiver:
This waiver provides Medicaid healthcare services to individuals aged 21 and younger with a physical disability who would not normally qualify for Medicaid due to family income.

Long-Term Care Insurance:
Long-term care insurance helps cover the costs of long-term Services & Supports at home, in an assisted living facility or in a nursing home.

Reverse Mortgages:
A reverse mortgage is a type of home loan older homeowners can use to tap accrued equity in their house for cash. Recipients can use the money to pay off debts, supplement other income, or pay healthcare expenses.
